SMS Terms & Privacy

1. Program Description

SOR Physical Therapy, operated by Sport Orthopedic Rehabilitative Services, Inc. ("SOR," "we," "our," or "us"), operates an SMS text messaging program to communicate with patients and prospective patients. Our text messaging program is used to send:

• Appointment reminders and confirmations
• Follow-up messages related to your physical therapy care
• Scheduling updates and cancellation notifications
• Health tips, wellness information, and general practice updates
• Responses to questions you send us via text
• Important notices about our services or office operations

SMS messages are sent from our practice's designated messaging number. This program is provided as a convenience and does not replace direct communication with your healthcare provider for urgent or emergency matters.

2. How to Opt In

You may enroll in our SMS program by:

• Checking the SMS opt-in box on a patient intake or contact form (paper or digital)
• Verbally consenting to receive text messages when registering as a new patient
• Texting a designated keyword to our SMS number as prompted on our website or marketing materials
• Providing your mobile number and checking the SMS consent checkbox on our website contact or appointment request form

By opting in, you confirm that:

• You are the owner or authorized user of the mobile number provided
• You consent to receive recurring text messages from SOR Physical Therapy at the number provided
• You are at least 18 years of age, or the parent/guardian of a minor patient

Your consent to receive marketing SMS messages is not a condition of receiving physical therapy services or medical care from SOR Physical Therapy.

3. Message Frequency

Message frequency varies depending on your appointment schedule and engagement with our practice. You may receive:

• Appointment reminders: typically 1–2 messages per scheduled visit
• Follow-up check-ins: 1–2 messages following a visit or treatment milestone
• General practice updates or health tips: up to 4 messages per month

We will not send unsolicited or excessive messages. You can opt out at any time (see Section 4 below).

4. How to Opt Out

You may opt out of our SMS program at any time by:

• Replying STOP to any text message you receive from us
• Replying UNSUBSCRIBE, CANCEL, END, or QUIT — these keywords all trigger opt-out
• Contacting us directly at (219) 777-3698 or [email protected] to request removal

After opting out, you will receive a single confirmation message acknowledging your opt-out. You will not receive any further SMS marketing messages following that confirmation. Note that opting out of marketing texts does not affect essential appointment reminders unless you specifically request that as well.

You can re-enroll at any time by texting START or contacting our office.

6. Message & Data Rates

Message and data rates may apply. Text messages sent to and from SOR Physical Therapy may be counted toward your mobile carrier's messaging plan. Charges depend on your individual carrier plan and data agreement.

SOR Physical Therapy is not responsible for any fees charged by your mobile carrier for sending or receiving text messages. Please contact your carrier for details about your plan.

7. No Third-Party Sharing for Marketing

We will not share, sell, rent, or trade your mobile phone number or SMS opt-in consent with any third party for their marketing purposes.

Your phone number may be shared with our trusted SMS technology platform and communication service providers solely to facilitate message delivery on our behalf. These providers are contractually prohibited from using your information for any purpose other than delivering our messages.

8. Privacy & Data Use

The information you provide when enrolling in our SMS program — including your name, mobile number, and any content you share via text — is handled in accordance with our Privacy Policy.

As a healthcare provider, SOR Physical Therapy is also subject to HIPAA. Any health-related information you share via SMS in connection with your care will be treated as Protected Health Information (PHI) consistent with our Notice of Privacy Practices.

We retain SMS opt-in records as required by applicable law and carrier regulations. You may request deletion of your personal information at any time, subject to legal retention requirements, by contacting us at [email protected].

9. Supported Carriers & Limitations

Our SMS program supports all major U.S. wireless carriers, including but not limited to AT&T, T-Mobile, Verizon, and their affiliates. Delivery of SMS messages is subject to carrier network availability and may be affected by technical issues outside our control.

SOR Physical Therapy and participating carriers are not liable for delayed or undelivered messages. If you experience delivery issues, please contact us directly by phone or email.

This program is available to U.S. mobile numbers only.
